A working pump and standing water is a contradiction worth resolving rather than dismissing, and there are only a few ways both can be true. Water may be arriving faster than the pump's rate — high water table, a storm event, or a discharge that recirculates back into the crawl. The pit may be positioned or set too high relative to the crawl's low points, so water pools where the drainage does not lead to the pit. The perimeter drainage feeding the pit may be blocked or absent, so water in the crawl never reaches the pump at all. Or the discharge may be obstructed downstream, so the pump runs and moves nothing. The inspector reports the standing water as a condition, notes that the pump operated, and identifies that the system as a whole is not managing the water — which is the finding, regardless of which of those explains it.
