Why this is the answer
Reg 68 (11 NYCRR § 65-1.1) requires health-care providers — and assignees — to submit no-fault medical bills on Form NF-3 within 45 days after the date services are rendered. Bills outside the 45-day window may be denied unless the provider demonstrates clear and reasonable justification for late submission. The 45-day clock runs separately for each service date. Hospital bills enjoy a 180-day extension.
