Behind a cover plate is an energised device with exposed terminals, and removing the plate puts the inspector's hands and tools near them. That is a live-parts hazard, and it is the reason plate removal is done selectively rather than routinely: where there is a specific reason to look, where the circuit's condition makes it reasonable, and where the inspector is equipped to do it safely. It is not prohibited by the standards, but neither is it a default step, and much of what would be learned — polarity, grounding, ground-fault protection — is obtainable from a receptacle tester without opening anything. Where a plate is removed the same discipline applies as at the panel: know what is energised, keep clear of terminals, and stop if conditions are worse than expected. Judgment about when to open is part of the competence being tested.
