Under 215 ILCS 5/143, no policy of insurance may be issued or delivered in Illinois unless the policy form has been:

Filed with and not disapproved by the Director of Insurance
BApproved in advance by the policyholder
CRegistered with the Illinois Secretary of State
DNotarized by an IDOI-appointed notary

Why this is the answer

Section 143 of the Illinois Insurance Code sets the form-filing framework. Insurers must file policy forms with the IDOI Director, who may disapprove forms that violate Illinois law, are deceptive, or contain prohibited provisions. For most P&C lines Illinois follows a file-and-use framework. The Director's affirmative approval is generally not required, but disapproval can occur within the statutory review window. This regime ensures Illinois insureds receive forms compliant with mandatory provisions, while preserving market efficiency.
